Designed for a high-information ICU handoff
Critical-care assignments generate dense information: airway and respiratory support, neurological findings, hemodynamics, access devices, continuous infusions, laboratory trends, procedures, safety concerns, and goals. This ICU shift report sheet gives nurses and nursing students a consistent visual framework for collecting that information and identifying what needs follow-up.
The worksheet supports personal organization before, during, and after handoff. It does not replace bedside verification, the EHR, MAR/eMAR, current orders, approved flowsheets, or your unit’s handoff standard.
What the ICU report sheet helps you track
- Neurological, respiratory, cardiovascular, gastrointestinal, genitourinary, skin, and mobility observations
- Airway support, oxygen settings, lines, tubes, drains, and devices
- Continuous infusions and time-sensitive therapies
- Safety risks, precautions, code status, allergies, and escalation concerns
- Key laboratory and diagnostic trends
- Shift goals, priority tasks, provider communication, and unresolved work
How to use it safely
Use the sheet as a prompt during report, then verify every clinically important detail in the official record and at the bedside. Record only information permitted by your employer. Update the sheet when the patient’s status changes, and follow your organization’s secure disposal policy at the end of the shift.
